The Eugene Guevremont Memorial Scholarship Program

Below are nomination instructions and some suggestions to guide you through the process.

PLEASE READ THESE INSTRUCTIONS CAREFULLY AND COMPLETELY!

Step 1.)   In choosing the one nominee for your campus, please consider the student’s academic
performance, record of community service and the impact this award will make on their
ability to complete their education. Note: We allow each campus to determine how they
wish to identify their nominee. Please give this process your careful consideration.

Step 2.)   Please make sure that the potential nominee meets the following eligibility requirements:

• Must be nominated by a high school teacher or administrator
• Must be a U.S. citizen or permanent resident
• Must have demonstrated academic success (with a minimum GPA of 2.75 on a 4.0 scale)
• Must have engaged in community service within the previous 12 months as validated by his/her nominator either on campus or in the community (Note: There is no minimum requirement however, we hope that you will nominate a student who are consistently active in the community) or is employed full or part-time.
• Must be planning to enroll in at least two courses during the upcoming fall term at a minimum of a two-year institution

Step 3.) Complete the Nomination Verification form and mail it to:

Nomination Verification forms without the designated high school’s contact’s signature will be considered invalid and will not be accepted. If you have any questions, please contact our office.

Please note that applications must be received BEFORE the midnight May 2nd 2008 DEADLINE.
